GUIDELINES ON THE ACKNOWLEDGMENT AND AUTHENTICATION OF DOCUMENTS

1. TWO MODES OF AUTHENTICATING DOCUMENTS:

1. Acknowledgment
   Interested party/parties must sign before a consular official of the Consulate.

Requirements:

• • • photocopy of State ID/ driver's license and
    • original and one (1) photocopy of each set of document to be
    • acknowledged or authenticated.

2. Authentication

1. If the interested party/parties CANNOT sign before the consular official, the document must be notarized by a local NOTARY PUBLIC.
2. The signature of the notary public must be authenticated by the COUNTY CLERK where the notary public is registered or by the SECRETARY OF STATE of the State, under the jurisdiction of the Consulate General, where he or she resides before submitting to the Consulate (Only the following states fall within the jurisdiction of the Consulate General: Arkansas, Illinois, Indiana, Iowa, Kansas, Michigan, Minnesota, Mississippi, Missouri, North Dakota, Nebraska, South Dakota, Ohio and Wisconsin).

Requirement: original and one (1) photocopy of each set of document to be authenticated.

Authentication Fee - Under Department Order 23-92, the fee for authentication and acknowledgment of a deed or power of attorney with one or more signatures is $25.00 per set of document. The applicant should pay for the corresponding number of sets of documents that he or she actually needs. Payment can be made through cash or Money Order payable to the Philippine Consulate General. Personal checks are not accepted.

Application by mail - If the applicant wishes the documents to be mailed by the Consulate, a self-addressed stamped priority/certified/express mail envelope or prepaid UPS/DHL for return of documents is required.

2. Additional Requirements for Patent and Trademark Documents:

Additional requirements for trademark; patent application; combined oath; corporate power of attorney and petition for trademark and patent; affidavit of non-use; appointment of resident agent, Certificate to Foreign Governments and assignment of letters of patent: the foregoing Document must also be authenticated by the SECRETARY OF STATE OF THE UNITED STATES with address at:
