PASSPORT
The Philippine Consulate General processes applications for passport. They are transmitted to the Department of Foreign Affairs in Manila for printing. Please allow two (2) months processing time from the date of application up to the date of release of the new passport to the applicant. The Philippine Passport is at all times the property of the Republic of the Philippines. It must be surrendered upon demand by an authorized representative of the Philippine Government. Alteration, addition, or destruction of contents is strictly prohibited. Any unauthorized change will render a passport invalid and will have accompanying penalties. Using the passport as collateral is against the law and will automatically render the passport cancelled. The information provided in the passport application form must be true and correct. Applicants should bring complete requirements, which should be authentic. Under the law, making false statements in the application form, furnishing falsified or forged documents in support thereof are punishable by law. Also, the Consulate reserves the right to request additional requirements, if necessary.
Important Reminders:
- - PERSONAL APPEARANCE IS REQUIRED at all times and application is by appointment only.
- - For application at the Consulate, please secure an appointment via the Department’s Online Appointment System by accessing the following link: ttps://www.passport.gov.ph
- - Applicants are advised to be at the Consulate’s premises not earlier than 10 minutes of their appointment schedule.
- - Photos will be taken at the Consulate (Do not bring your own photos).
- - For photo capturing, applicants are required to wear a decent attire. Jewelry, piercings, glasses and colored contact lenses will not be allowed.
- - Bring photocopies as required. The Consulate does not offer photocopy services.
- - Bring your own pen and observe applicable health protocols.
- - For the delivery of new passports, each applicant must BRING a self-addressed envelope (preferably USPS Priority Mail or UPS Prepaid envelope, with stamps and tracking number) except FEDEXThe Consulate General assumes no responsibility for any loss or damage through mail.

Requirements
RENEWAL
- 1. Print Passport Application form attached on your appointment confirmation.
- 2. Original Current Passport and photocopy of data page (and photo page for old brown passport)
- 3. Photocopy of green card or US visa
- 4. For Dual Citizens, photocopy of Identification Certificate
- 5. Additional requirements for minor applicants, and for female applicants who will use their husbands’ last name for the first time or who will revert to their maiden name (Please see below)
Additional Requirements for non-ePassports (Green Passports, and Passport No. starting with XX and other older Passports)
- 1. Original and photocopy of valid ID:
- • Driver’s license / State ID
- • School or Work ID
- • Philippine ID (SSS card, Senior ID, School ID, PWD ID or Voter’s ID)
- 2. For older passports with incomplete details (i.e. no Middle Name):
- • Original and photocopy of PSA-issued Birth Certificate

REPLACEMENT OF LOST PASSPORT

- 1. Personal appearance of Parent/s, Guardian, or authorized adult companion
- 2. Original and photocopy of Proof of Filiation or Guardianship (e.g. Birth Certificate, Baptismal Certificate, Court Documents etc.)
- 3. Original and photocopy of PSA-issued Marriage Contract if the father is accompanying the child
- 4. Notarized Special Power of Attorney (SPA) executed by the mother (notarized at a PHL Embassy/ Consulate or apostillized) authorizing the adult companion (including the father if not married with the mother) if the mother is not accompanying the child
- 5. Original and photocopy of passport/ valid ID of parent, guardian or authorized adult companion
ADDITIONAL REQUIREMENTS FOR FEMAL APPLICANTS WHO WILL USE THEIR HUSBAND'S LAST NAME FOR THE FIRST TIME AND OR WHO WILL REVERT TO THEIR MAIDEN NAME
- 1. For applicants who will use their husbands’ last name for the first time:
- • Original and photocopy of PSA-issued Marriage Contract or Report of Marriage (ROM). If ROM was issued more than a year ago, PSA-issued ROM
- 2. For applicants who will revert to their maiden name:
- • For widowed applicants: Original and photocopy of PSA-issued Death Certificate of husband, and of Marriage Contract/ROM
- • For annulled marriages: Original and photocopy of PSA-issued Marriage Contract with Annotation on Annulment
- • For divorced applicants: Original and photocopy of authenticated Judicial Recognition of Divorce (issued by a PHL court), and of PSA-issued ROM
WALK-IN SLOTS
The following passport applicants may be accommodated as walk-ins (even without an appointment) on work/week days (except Fridays and holidays):
- 1. Applicants who are 60 years old and above;
- 2. Minors who are 7 years old and below (with an adult companion);
- 3. Applicants with disability (must present a Disability ID);
- 4. Pregnant applicants (must present medical records); and,
- 5. Applicants with an immediate need of a passport (must present proof of urgency, such as a death certificate of a family member, an expiring stay/work permit and the like).
For walk-ins, please use the appropriate Application Form in the “Downloadable Forms.” Accomplish and bring the same when you proceed to the Consulate.
The Consulate General, during the processing of applications, will prioritize applicants with confirmed appointments.
